Insurance Information Form

Watkins will submit claims to any insurance company although we are only in-network with certain carriers. We recommend that you have a copy of your insurance card with you at all times.  If you do not have a card for your insurance, we recommend keeping a completed Insurance Information Form with you at all times. You may request a printable version of our Insurance Information Form by emailing our Business Office at whs-billing-insurance@ku.edu. Upon your first visit to Watkins, we will ask for this information.
