Immunization History Form

KU requires immunization records for all new students be submitted to Watkins Health Services.

Watkins Health Services will accept immunization records from a personal physician's office, school, military, state record, or childhood immunization booklet as official documentation. However, if you do not have one of these official records, you can use this Immunization History Form (pdf); but it is required to be filled out and signed by your personal physician's office.

All documentation submitted to Watkins Health Services will be verified.

U.S. Students

Seventy-two (72) hours after enrolling at orientation, you will be able to log into the Watkins Health Services patient portal. Click on the "Medical Clearances" tab and select "Immunization Upload" to upload your form. 

 

International Students

Prior to your arrival at KU, consult with your personal physician's office to complete the Immunization History Form (pdf). The form must be completed in English and signed by your doctor. After you schedule your health check-in appointment, log into Watkins Health Services patient portal. Click on the "Medical Clearances" tab and select "Immunization Upload" to upload your form.
