Visit Notes Now Available
As of April 5, 2021, our patients have immediate access to more information in their electronic medical record. In compliance with new federal guidelines of the 21st Century Cures Act, clinical notes are available to patients through their secure online portal following their visit. Information sharing provides patients with timely access to their health data and empowers patients and their providers to make more informed decisions. Patients play an important role when they serve as part of their own care team, help to ensure accurate data and can close information gaps.
Per this new law, the eight types of clinical notes that must be shared include:
- consultation notes
- discharge summary notes
- history and physical
- imaging narratives
- laboratory report narratives
- pathology report narratives
- procedure notes
- progress notes
Clinical notes to which the rules do not apply:
- Psychotherapy notes. The rules about what must be shared are not quite as extensive when it comes to psychotherapy notes. While clinicians will still be required to share information pertaining to things like prescriptions, treatment furnished, results of clinical tests, symptoms, treatment plan, or diagnosis, any recorded notes that are separated from the rest of the individual’s medical record will not be required to be disclosed.
- Notes gathered in anticipation of legal proceedings. This includes all information compiled in reasonable anticipation of, or use in, a civil, criminal, or administrative action or proceeding.
For current patients, logging in to your secure patient portal will allow you to see additional clinical results and clinician notes as soon as they’re complete by clicking on Medical Records from the left menu. This means that you may see your results before your provider has had a chance to review them. These results could include sensitive or difficult to understand information.
